How to Receive Your Rebate Check
1. Get a contact lens fitting and eye exam. 2. Within 90 days of your exam date, purchase the required quantity of lenses from the list above. You will receive your mail-in rebate with your contact lenses. 3. Follow the instructions on the rebate form.* *Offer valid in the U.S. and Puerto Rico only. These rebate offers are subject to the Terms and Conditions on the rebate form. Valid on purchases January 1, 2010 through July 31, 2010. Rebate submission must be postmarked by August 31, 2010.
